As a member of a multidisciplinary team in a 16-bed bed inpatient crisis unit, provide/coordinate for residents services, programs, and treatment that are within the scope of the Social Work profession.
Job Duties- Formulate and execute discharge plans.
- Assist in the admission process.
- Provide psycho-educational experiences.
- Communicate with support systems (families, friends, group homes).
- Communicate with providers.
- Coordinate placement.
- Serve on treatment team.
- Collect and submit Medicaid prior approval information.
- Maintain documentation.
- A minimum of a Bachelor's Degree in Social Work and passage of the Social Work Licensure Test with Social Work License current and in good standing.
- Valid Mississippi Driver's License
