Schedule:
Full Time / Days
- Plan and coordinate new referrals.
- Provide phone triage for existing patients.
- Perform onsite office duties.
- Provide direct nursing services and case management related to infusion therapy for patients in the home, clinic, or alternate living sites.
- Collaborate with providers, patients and/or caregivers, physicians, and payors to ensure coordinated care.
- Serve patients in Oregon and/or southwest Washington.
- Oregon Registered Nurse license or Washington Registered Nurse license upon hire.
- National Provider BLS – American Heart Association certification within 30 days of hire.
- License in all states where care is delivered.
- At least 3 years of home infusion nursing experience.
- Bachelor's Degree in Nursing.
