Overview
Speech Language Pathologist is a licensed professional who provides direct and indirect patient care services to patients in their home environment or alternative setting according to the organization’s policies and procedures. The role includes evaluating patients, developing speech therapy treatment programs in collaboration with physicians, administering complex active and passive therapeutic exercises, and instructing, motivating, and assisting patients and caregivers in learning and improving activities of daily living.
Responsibilities- Assess and analyze patient care needs, establishing mutual goals with patients and/or caregivers for a comprehensive plan of care, including discharge planning.
- Assess the patient’s environment to ensure safety and suitability for effective care delivery.
- Evaluate the patient’s support system and utilize community resources for optimum care level.
- Identify therapy diagnoses and patient care problems, making necessary interventions and documenting progress.
- Interpret and implement Physician’s orders related to Speech Therapy, ensuring professional and ethical compliance with state and federal regulations.
- Coordinate multidisciplinary care plans and integrate services as needed.
- Document changes to the patient’s care plan and re‑evaluate progress, adjusting therapy under physician direction.
- Provide patient and caregiver education on diagnosis, medication, infection control, and safety measures.
- Conduct comprehensive medication reviews, ensuring families understand regimen details.
- Communicate with other disciplines to enhance patient outcomes.
- Serve all aspects of case management to improve outcomes.
- Complete and submit required documentation within agency guidelines.
- Ensure compliance with state, local, and payer requirements, meeting company and regulatory standards.
- Request and manage equipment and supplies that increase client function and independence.
- Demonstrate teaching skills and competence in therapeutic interventions across complexity levels.
- Coordinate care delivery with internal and external providers.
- Teach safety measures to patients and caregivers.
- Maintain universal precautions and infection control standards, properly sanitizing equipment and disposing of medical waste.
- Recognize and report changes in patient status appropriately.
- Provide clear, concise, and complete patient status reporting in compliance with agency and regulatory standards.
- Complete documentation with accuracy and timeliness.
- Demonstrate proficiency in OASIS documentation.
- Report all incidents per agency and regulatory standards.
- Adhere to policies and procedures per agency, state, and regulatory standards.
- Use safe body mechanics to prevent injury.
- Work within scope of practice.
- Attend staff meetings, care conferences, in‑services, and other meetings as requested.
- Maintain patient confidentiality and honor patient rights.
- Participate in the agency’s QAPI program, including data collection and performance improvement projects.
- Perform other duties within scope of practice as required.
- Master’s or Doctoral Degree in Speech Language Pathology from an accredited school.
- Valid state license to practice.
- Current CPR certification.
- Prior home care experience preferred.
- Ability to administer manual therapeutic exercises and guide non‑manual functional activities.
- Ability to evaluate cognitive, swallowing, expression, understanding, and fluency, developing care plans based on findings.
- Professional appearance and demeanor.
- Basic computer knowledge and ability to learn industry‑specific software.
- Excellent oral, written, and interpersonal communication skills.
- Excellent organizational and time‑management skills with keen attention to detail.
- Team player with effective collaboration skills.
- Critical thinking, research ability, and problem‑solving skills.
- Willingness to learn and complete educational goals during orientation.

The successful candidate will initially be employed by LHC Group until January 1, 2026, at which point employment will transfer to Adoration Home Health / Bright Spring Health Services.
