The Hospital Discharge Team are based at the Diana Princess of Wales Hospital. The Hospital Discharge Team is responsible for the safe and effective discharge of patients from hospital into the community and also the prevention of admission to hospital.
The Hospital Discharge Team works in partnership with the Acute Trust ensuring effective prevention and discharge pathways are followed. Thepostholder will provide professional support to those staff working in the Hospital Discharge Team.
The team operates 7 days per week working between thehours of 8.00am and 8.00pm.

Youwill be expected to work as part of the HDT team under the direction ofthe Clinical Lead, to ensure the smooth flow of patients through the team. It is expected that by coordinating administrative tasks, and liaising with other departments, you will contribute to the efficient and effective running of the Discharge Team resulting ina significant reduction inpatient journey times and an effective discharge plan beingin place.

Person Specification Experience- Minimum of 6 months administration/reception experience.
- Experience of data input, spread sheets/statistics.
- Competent in Microsoft office including Word, Outlook & Excel
- Stock control
- Diary management
- Servicing meetings
- System One
- Experience of working with people with varying levels of communication skills/abilities
- Experience of working in a care environment
- Supervisory experience
- Ability to effectively manage own workload.
- Accurate data input and word processing.
- Accurate minute taking.

- Ability to liaise effectively with people at all levels.
- Effective customer care skills.

- Literacy and numeracy level 2 qualification
- Level 3 or equivalent qualification in computer applications
- Level 2 NVQ Business Administration

- Ability to work flexibly regarding working hours including weekends/arrangements and venues
