Care Plus Group Primary Care practices provide the first point of contact for registered patients accessing the health care service. The service will work collaboratively with patients to assist them in managing and preventing illnesses and coordinating access to specialised care.

The Care Plus Group Primary Care practices, Open Door and Quayside, provide services for a client group with a range of diverse and sometimes complex needs including substance misuse, homelessness, mental health problems and self-neglect.

22.5 hours per week

Rota Pattern:
Monday Friday

Permanent Contract

£24,853 pro rata per annum fixed salary point

The aim of this service is to improved childhood immunisation and general health amongst the hard to reach and other priority groups across North East Lincolnshire to reduce the spread of childhood disease; this will be achieved by supporting parents and building relationships within their GP practice and/or offering flexible arrangements for vaccinations as well as providing general advice and awareness around childhood illnesses.

The service is for children and families within North East Lincolnshire who do not consistently engage with health services. It enhances the service already provided by GPs across NEL by assisting them to follow up children who have continually missed routine childhood vaccinations.

By working closely with the community we aim to increase immunity of childhood illness, create personal responsibility and reduce reliance on Secondary Care services such as Emergency Care Centre and hospitals.

Qualifications
  • NVQ 3 or above in Health and Social Care or equivalent.
Other
  • The individual will require transport to enable them to facilitate transport of self and service users in line with the needs of the service e.g. transporting service users to appointments and visiting Service Users in their own homes around North East Lincolnshire.
  • Ability to work flexibility to support service needs.
  • Ability to show commitment to Care Plus Group core values.
Experience
  • Previous experience or working directly with people within a recognised field E.G Health, Education, Housing, Social Services, Voluntary Sector or Youth and Community.
  • Knowledge of immunisations, general health, wellbeing, sexual health, self-harm, substance misuse, the interface between primary & secondary care.
  • Effective organisational skills and effective time management.
  • A clear understanding of the inequalities and causes of poor health outcomes of people.
  • Understanding health promotion principles and practices.
  • Experience of working within a multi agency settings.
  • Skill in meaningfully involve hard to reach people.
Knowledge & Skills
  • Ability to recognise own limitations and act upon them appropriately.
  • Willingness to learn new skills and to problem solve on a daily basis.
  • Ability to travel around North East Lincolnshire.
