What You Will Do
- Advanced Clinical Assessments:Provide triage and clinical assessments at the
mental health Single Point of Access
, ensuring smooth and efficient entry into crisis services. - Interventions & Preventive Care:Implement brief interventions to avoid unnecessary hospital admissions or visits to A&E, promoting early intervention and support.
- Collaboration with Agencies:Build strong relationships with key partners such as
police
,
ambulance services
, and local agencies to enhance support networks and improve service delivery. - Mental Health Awareness:Contribute to
training
and
mental health awareness
initiatives, particularly for acute hospital staff. - Care Plan Development:Work closely with service users and their networks to create
safe, compassionate, and evidence-based care plans
