Mental Health Practitioner

An exciting opportunity has arisen for a highly motivated and enthusiastic Mental Health Practitioner to join the Care Home Liaison Team

Please note the earliest start date for this position is 1st February 2027.

Our ideal candidate would be acommitted team player as well as being able to work independently,demonstrating initiative and be passionate in promoting good quality care to maximise independence. Within this role you will be responsible for assessing,developing, implementing and evaluating care plans using a person-centred approach. Within this role you will gain training, support, and guidance toensure you are working collaboratively with external agencies to provide thebest possible care for our service users.

This role requires someone whocan manage in a fast-paced environment completing assessments to support diagnosis. The day-to-day work - for this role will be led by the needs of the service users and the service which will require effective time management and prioritisation skills as well as effective communication and autonomous working.

All of our staff are an integral part of the multidisciplinary team within mental health services.

• Communication & relationship skills

  Use communication skills to conduct assessments of service-users and communicate complex information related to their mental health needs, to service-users,carers, relatives and other professionals.

  Effectively communicate complex and/or sensitive information where there are barriers to understanding. This includes communication concerning adult protection, abuse,cognitive impairment and other risks surrounding a service-users mental health. Barriers to understanding include: working with service-users who couldbe hostile or highly emotional, who have complex needs and may be vulnerable.

  Maintain and develop effective communication and working relationships within the teamand with external agencies.

  Promote and implement creative ways of working with service-users.

  Instigate,develop and maintain a relationship with the client and / or carer to facilitate the accurate assessment and delivery of care.

  Manage highly confidential information in respect of members of the public in accordance with local and national policy.

  Actas care co-ordinator and named worker responsible for specialist assessment,reviewing and on-going care.

• Knowledge,training and experience

  Thepost-holder will hold current registration on part 3 or 13 of the NMC register or hold a Registered Occupational Therapist, Social Worker, Mental Nursing(RMN) certificate.

  Havepost registration experience and / or training, relating to older people andmental health and apply this knowledge when delivering care to service-users.

  Provide advice and support to other agencies around issues relating to mental health.

• Analytical & judgement skills

  Under supervision make decisions about the range of options for treatment and careplans based on judgement of service-users need.

  Conduct comprehensive assessments of both service users and carers in line with team operational policies.

• Planning & organisational skills

  Planand co-ordinate care plan review meetings with colleagues in the team and withstaff in other teams and agencies, ensuring that review meetings take place atthe appropriate time.

  Co-ordinate transfer or referral of service-users to other teams or services where required.
