North Thoresby and Holton-le-Clay Medical Practice are seeking an experienced, motivated and patient-focused Diabetes Specialist Nurse / Practice Nurse to join our friendly and supportive team.
Main duties of the jobThis role is ideally suited to a registered nurse with significant experience in diabetes management within primary care and who is insulin trained. Alongside leading diabetes care, the successful candidate will provide a range of general practice nursing services and support the management of long-term conditions.
The post holder will play a key role in improving patient outcomes through evidence-based care, patient education, and collaborative working with the wider multidisciplinary team.

Job responsibilitiesMain Duties of the Role
The successful candidate will:
- Lead and support the management of patients with diabetes in accordance with current clinical guidelines.
- Conduct diabetes reviews, insulin initiation and monitoring where appropriate.
- Provide education and support to patients to promote self-management and improve health outcomes.
- Support the achievement of Quality and Outcomes Framework (QOF) targets relating to long-term conditions.
- Deliver a range of general practice nursing services including cervical screening, immunisations, wound care and health promotion.
- Work collaboratively with GPs and the wider multidisciplinary team to provide high-quality patient care.
- Maintain accurate clinical records and contribute to service development and quality improvement initiatives.

Person Specification Qualifications- Registered Nurse with current Nursing (NMC) registration.
- Recognised qualification, training, or substantial experience in diabetes management.
- Insulin initiation and management qualification/training.
