Target Housing is looking for an AST Support Worker to join our team in Grimsby on a 6-month fixed-term contract, providing maternity cover.

This is a rewarding opportunity to support vulnerable adults to develop the skills, confidence and independence they need to move towards more independent accommodation and achieve their personal goals.

You’ll work alongside tenants to help them sustain their accommodation, manage their finances, access benefits, develop independent living skills and overcome the challenges they may face.

About the Role:

As an AST Support Worker, you will provide person-centred housing-related support to tenants living in Target Housing accommodation. You’ll act as a key worker for a caseload of tenants, building trusting and professional relationships and working with each person to understand their circumstances, aspirations and support needs. The ultimate aim is to help tenants develop the skills and confidence they need to sustain their accommodation and move on to greater independence.

What

You’ll Be Doing:
  • Supporting tenants when moving into or out of Target Housing accommodation.
  • Assessing tenants’ circumstances, aspirations and support needs and developing personalised support plans.
  • Acting as a key worker and providing practical advice, guidance and emotional support.
  • Helping tenants develop the skills needed for independent living.
  • Supporting tenants to maximise their income and manage personal budgets, including accessing welfare benefits.
  • Helping tenants understand and meet their rights and responsibilities under their occupancy agreements.
  • Motivating and encouraging tenants to work towards their personal goals and greater independence.
  • Completing and regularly reviewing risk assessments and safety plans.
  • Developing, reviewing and updating support plans to demonstrate progress against agreed outcomes.
  • Supporting tenants to understand and meet their financial responsibilities, including rent and household bills.
  • Helping tenants develop the skills needed to maintain their accommodation and manage household utilities such as gas and electricity.
  • Monitoring compliance with occupancy agreements and addressing concerns proactively.
  • Monitoring properties for potential abandonment or damage and taking appropriate action when required.
  • Working collaboratively with partner agencies to provide coordinated support.
  • Managing and proactively addressing neighbour disputes while encouraging tenants to become positive members of their communities.
  • Assisting with the delivery and development of tenant activities.
  • Building positive, professional relationships with tenants, colleagues and external organisations.
  • Maintaining accurate, factual and up-to-date records in accordance with contractual requirements.
