Een zorginstelling in Groningen zoekt een trajectbegeleider voor 28-32 uur per week. Je begeleidt 27 bewoners met complexe problematiek en werkt aan hun doelen. Een MBO niveau 4 diploma in Maatschappelijke Zorg is vereist. Je rapporteert over je werkzaamheden en signaleert knelpunten in de hulpverlening.
