Je neemt een voortrekkersrol in zorgcoördinatie bij complexe casuïstiek en begeleidt de implementatie van Triple-C en methodisch werken binnen het team. Je werkt in een multidisciplinair verband en draagt bij aan kwalitatieve zorgontwikkeling.
