Zorgplein Noord zoekt per 1 augustus een Trajectbegeleider B in Groningen voor 24-32 uur per week. Je biedt zelfstandige begeleiding aan cliënten met complexe problemen en ontwikkelt maatwerk-begeleidingsplannen. De organisatie heeft een open bedrijfscultuur waar je betrokken bent bij zowel cliënten als je team.
Je hebt minimaal een HBO-diploma en ervaring in de maatschappelijke zorg. Aantrekkelijke secundaire arbeidsvoorwaarden zoals een individueel keuzebudget en een loopbaanbudget worden aangeboden.
