De Zijlen zoekt een betrokken persoonlijk begeleider voor de locatie De Driemaster in Lewenborg. Je werkt met jongeren (15-23 jaar) aan zelfstandig wonen en leert hen hun doelen te bereiken.
Je begeleidt de groep, onderhoudt ouders contact en werkt nauw samen met het netwerk en verschillende disciplines. Een vaste 32-urige werkweek en een onbepaalde tijdsovereenkomst horen bij de functie.
