Bij Beter Thuis Wonen werkt de Ambulant Begeleider A in de eigen omgeving van cliënten en coördineert zorgplannen zodat zelfstandigheid en participatie bevorderd worden. Je bezoekt cliënten thuis, coördineert begeleiding en schakelt met ketenpartners voor brede ondersteuning.
