Beter Thuis Wonen zoekt een ervaren Ambulant Begeleider A om cliënten thuis te begeleiden en zorg te coördineren bij complexe problematiek. Je bouwt aan een integrale aanpak die zelfstandigheid en participatie vergroot, met aandacht voor samenwerking met ketenpartners en sociale netwerken.
Je bespreekt signalen met de Coördinator en andere zorgverleners. Een rijbewijs en een auto zijn prettig. Er ligt een focus op professionele ontwikkeling via onze academie.
