WIJ Groningen zoekt een generalist 18+ die inwoners ondersteunt en Wmo-indicaties kan afwegen. Je bent het eerste aanspreekpunt en werkt aan versterking van zelfredzaamheid door passende begeleiding te bieden of door te verwijzen naar passende ondersteuning.
Je versterkt ook het netwerk van de inwoner en ondersteunt samenwerking met andere professionals. Het gaat om 16–24 uur per week, een arbeidsovereenkomst van 1 jaar met kans op verlenging.
