We’re seeking a motivated PTA to serve as a Community Liaison for our growing East Side home health agency. This unique role is perfect for a clinician who wants to blend therapy expertise with community outreach and relationship building.

• Represent our agency throughout the East Side, building partnerships with physicians, senior living communities, hospitals, and referral sources.
• Educate healthcare professionals, patients, and families about our home health services.
• Serve as a knowledgeable clinical resource to referral partners by explaining therapy programs and answering questions.
• Support the agency’s mission to deliver compassionate, high-quality care to patients in their homes.

• Current Physical Therapist Assistant (PTA) license in Michigan.
• Excellent communication, interpersonal, and presentation skills.
• Ability to balance clinical knowledge with relationship development.
• Self‑motivated, professional, and confident engaging with healthcare providers and the public.
• Prior experience in home health or outreach preferred, but not required.
