OverviewWork as an inpatient pharmacist on nights in Columbia County. The Pharmacist is responsible for appropriate, evidence-based medication use for the institution. They ensure patients get the right medication at the right dose. Pharmacists must have a medication safety viewpoint and be in compliance with all applicable standards and regulations (TJC, BOP, CMS, etc). Pharmacists oversee the activities of pharmacy technicians.
Pharmacists must engage in teaching as part of the hospital's mission.
- Provide clinical pharmaceutical care to patients. Prevent, detect, and resolve medication-related problems and make evidence-based, patient-centered medication therapy recommendations.
- Participate as an effective member of interdisciplinary care teams and communicate follow-up with the team. Verify patient medication orders for appropriateness and accuracy.
- Provide effective medication education to patients, families, and the care team. Contribute to quality of care delivery.
- Develop and implement performance/quality improvement and patient safety initiatives, including medication event/adverse reaction reporting and follow-up, clinical guideline development, disease management, and medication use evaluation/improvement with cost containment actions.
- Represent pharmacy services on institutional and practice area committees and quality unit councils. Conduct monthly unit inspections of assigned areas.
- Demonstrate proficiency in pharmacy distributive services and ensure quality, safety, and efficiency in the distribution process.
- Collaborate with pharmacy coworkers for communication, customer service, and safe drug distribution. Provide oversight for pharmacy technicians.
- Support education and professional development of the Pharmacy Department and other hospital disciplines. Precept residents, students, or technicians as assigned.
- Participate in development of new programs, services, and practices; engage in professional self-development; complete competency requirements by deadlines.
- Establish regular meetings with manager and attend seminars/continuing education to enhance knowledge/competency.
- Demonstrate active membership/service/leadership in professional organizations. Perform other duties as assigned. Comply with Wellstar Health System policies and code of conduct.
Minimum Education
- Bachelors Pharmacy or Doctorate Pharmacy – Preferred
- All certifications are required upon hire unless otherwise stated. RPH - Registered Pharmacist
- Minimum 2 years in Pharmacy as a pharmacy technician, intern and/or pharmacist – Preferred
