Build and Lead Care Coordination Operations at Wellstar’s New Columbia County Medical Center in Grovetown, GA!

• The Director of Care Coordination is responsible for the day-to-day operations of all case management activities across the hospital and for building strategies aligned with current performance, hospital goals, and the vision for the care management program.
• The Director of Care Coordination reports directly to the Assistant Vice President of Operations and oversees a team of 16 FTEs.
• This leader will be responsible for hiring staff for a new hospital opening, managing onboarding and training, creating and enhancing departmental policies, and participating in committee meetings to address and resolve issues.
• The ideal candidate will be an experienced leader with a proven track record of clinical excellence. Strong employee engagement, communication, and collaboration skills are essential.

• Bachelor of Science in Nursing and Master’s in Social Work required.
• Active Registered Nurse license required.
• ACCM certification required.
• Minimum of 3 years of experience in case management, quality, or patient safety, along with at least 6 years of experience in a hospital environment.

• Wellstar Columbia County Medical Center is a brand-new, state-of-the-art acute care hospital being developed to serve the rapidly growing Columbia County and Greater Augusta region, expanding access to high-quality healthcare close to home.
• The six-story campus is designed to include approximately 100 inpatient beds, a modern Emergency Department, six operating rooms, advanced imaging capabilities, interventional services, and a heliport to support critical patient transport.
• An adjacent medical office building will expand outpatient and specialty services, supporting a seamless continuum of care while addressing long-standing healthcare access needs in the community.
