POSITION SUMMARY

The Person al Support Workers – Hospital at Home Waterloo Wellington Program (PSW-HHWW) enhance the quality of life of SJHC clients by providing personal care assistance, medication reminders, physical and cognitive support, and home keeping assistance. PSW-HHWW helps clients live safely and healthily after hospital discharge and supports daily activities of living.

PSW-HHWW works cooperatively with the program Director, Managers, Clinical Educators, Program Assistants and other multidisciplinary care givers in delivering client-centered care. PSW-HHWW adheres to daily care schedules and overall care plans. Clients have recently been discharged from hospital and require support in daily care management and activities. There may be opportunities for enhanced learning and skills transfer with certain client populations.

CORE

DUTIES AND RESPONSIBILITIES
  • Assisting clients with daily personal care activities according to individual care plans (Bathing;
    Dressing/undressing;
    Personal hair care;
    Shaving/grooming; Mouth care; Preventative skin care; Bathroom activities; Feeding; Other personal hygiene tasks)
  • Assisting clients with daily housekeeping activities (Light apartment housekeeping; Laundry services; Light meal preparation / safe food handling)
  • Assisting clients in daily activities to maintain mental/physical health and safety (Performing safety wellness checks;
    Medication reminders / administration;
    Assisting with medical devices; Living area visual scans; Engaging clients in positive conversations; Encouraging client-directed care)
  • Assisting with mobility and participation in social/recreational activities;
    Encouraging positive life skills
  • Assisting with basic range of motion exercises;
    May be trained for additional tasks within scope of practice (e.g., peritoneal dialysis, hemodialysis)
  • Demonstrating sensitivity to clients with cognitive, physical, or mental health challenges;
    Providing emotional and social support to clients and families
  • Maintaining a positive, professional attitude and supporting co-workers; following care plans and program procedures
  • Demonstrating ability to follow directions, be detailed-oriented, and maintain privacy and confidentiality of health records
  • Participating in team meetings and quality improvement initiatives
  • Maintaining compliance with corporate policies, health and safety regulations, and external standards
QUALIFICATIONS
  • PSW Certificate from an accredited training institution, required
  • 1-3 years PSW experience, preferred
  • Member of the PSW Registry where required by SJHC contract, preferred
  • Current First Aid and/or CPR Certificate, preferred
  • Training in gentle persuasive/non-violent crisis prevention, an asset
  • Training and experience in Medication Reminders, preferred
  • Previous experience with respite care, required
  • Previous experience with Dementia Care and Dementia clients, preferred
  • Previous experience in community or home care setting, preferred
  • Registration with HSCPOA, an asset but not required
  • Ontario Driver’s License and vehicle, required
