Key Responsibilities:
• Assess resident nutrition status upon admission and annually
• Develop and modify individualized nutrition care plans
• Document all nutrition interventions clearly
• Participate in interdisciplinary care conferences
• Educate staff on dietary needs and nutritional care
Requirements:
• University degree in Food and Nutrition
• 1-2 years experience in long-term care preferred
• Current registration with provincial dietetic regulatory body
• Strong communication and critical thinking skills
• Proficient in Microsoft Office and Point Click Care
