Job Summary
Responsible for providing care to clients as well as supervision and direction to staff including and not limited to PSWs, support services, etc. on a designated unit. The RPN supports the mission, vision and values of SJHCG, as well as working towards established goals and objectives. The RPN is also responsible for following all policies, processes and procedures of SJHCG.
Key Accountabilities- Performs all duties in accordance with legislation, the Standards of Nursing Practice of the College of Nurses of Ontario, the College of Nurses Guidelines for Professional Behaviour, the Mission Statement, and policies and procedures of St. Joseph’s Health Centre Guelph.

- Performing nursing assessment of clients with medical and psychiatric needs, referring/collaborating with the Registered Nurse in performing nursing assessment of older adults with complex medical and psychiatric needs
- Detecting and documenting significant changes in a client’s condition
- Anticipating deterioration prior to explicit confirming diagnostic signs, and anticipating challenges and problem solving with others
- Understanding the particular demands of an illness and anticipating care needs
- Assessing the individual’s potential for wellness and for responding to various treatment strategies
- Effective Management of Rapidly Changing Situations
- Managing extreme life‑threatening emergencies before and after the arrival of the resuscitation team
- Identifying and managing a client in crisis
- Administering and Monitoring Therapeutic Interventions and Regimes
- Performing and monitoring vascular access (VA) therapy (hypodermoclysis)
- Promoting safety, comfort, hygiene, promoting respiration / oxygenation
- Administering medications
- Promoting fluid balance, nutrition and digestion, elimination, posture, mobility and ambulation
- Preventing and controlling infection
- Promoting skin integrity, neurological status, balance between rest and activity, psychosocial/mental/emotional function
- Providing behavioural care
- Obtaining, preparing, and preserving specimens
- Assisting with diagnostic procedures
- Planning and implementing nursing interventions, then evaluating the outcomes of nursing interventions for older adults with medical and psychiatric needs
- Supporting client/family access to health care and community resources
- Establishing and implementing education plans with and for clients and families
- Establishing and Maintaining Healthy Interpersonal Relationships
- Demonstrating enthusiasm for working with older adults and their families
- Developing and preserving therapeutic nurse/client relationships, developing and preserving collaborative relationships among members of the interdisciplinary team
