This role in the Rural Waterloo area enables you to provide nursing care that prioritizes dignity and privacy. With your RN diploma and active CNO registration, you'll collaborate with interdisciplinary teams to deliver exceptional services to patients both in-person and virtually, focusing on innovative health care practices.
Key Responsibilities:
• Develop individualized care plans in coordination with HCCSS
• Provide nursing services in clinics, schools, and homes
• Communicate effectively with patients and care team members
• Champion quality health care programs in the community
• Utilize full nursing practice for patient support
Requirements:
• RN diploma and registration with the College of Nurses of Ontario
• Valid driver’s license and capability for travel
• Excellent English communication, both verbal and written
• Required current immunization record with TB test
• Commitment to ongoing education and nursing certifications
