This pivotal role at Homewood Health Centre lets you utilize over three years of Nurse Practitioner experience to perform in-depth health assessments and create individualized treatment plans. Collaborating with physicians and a multi-disciplinary team, you will ensure that care meets professional standards and respects each patient's values. Key Responsibilities:
• Perform thorough health assessments and develop personalized care plans
• Collaborate with interdisciplinary teams on effective patient management
• Facilitate early assessment and intervention strategies
• Document patient care accurately in line with professional standards
• Nurture a therapeutic environment for patients and families
Requirements:
• 3+ years of NP experience, preferably in psycho geriatrics
• Active registration with the College of Nurses of Ontario
• Master's degree in Nursing necessary
• Membership in organizations such as NPAO or RNAO
• CPR certification and knowledge in crisis intervention
