• Health Force Ontario (HFO) RN, RPN, Return of Service (CCPN): $25,000 funding for eligible nursing graduates in exchange for a 24‑month commitment.

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and client’s unique needs.
• When designated as a primary provider for a client caseload, act as point of care lead, accountable for their care while collaborating with other team members and support functions.
• Support clients through care transitions by collaborating and connecting them with community support services in their local community.

• Current Certificate of Competence with the College of Nurses of Ontario as RN.
• Availability to work full‑time days, evenings, overnight shift and every other weekend.
• Updated immunisation records, clear criminal background check and Vulnerable sector check required upon hire.
• Willingness to attain required SE Health certifications.
• Current CPR certificate required.
• Physical demands: sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level, and hand grips of varying forces.
• Required to travel to Guelph, Fergus, Elora, Rockwood and surrounding areas.
• Trach and ventilation experience preferred.
