Position: Nurse Practitioner for Long Term Care
Join St. Joseph’s Health Centre Guelph as a Nurse Practitioner dedicated to holistic care in Long Term Care. Lead client assessments, develop care plans, and enhance medical standards in a collaborative setting.

In this nursing role, you'll provide comprehensive assessments and implement effective care for clients in a Long Term Care environment. You will work closely with healthcare teams and community partners to drive continuous improvement within the program. Your expertise in gerontology and leadership will be critical in shaping clinical standards and supporting nursing staff.

Key Responsibilities:

• Conduct comprehensive health histories and assessments

• Order diagnostic tests per medical directives

• Develop and document client care plans

• Collaborate with interdisciplinary teams for care planning

• Initiate urgent interventions as needed

Requirements:

• Current registration with the College of Nurses of Ontario

• Minimum three years as a Nurse Practitioner in LTC

• Advanced assessment and diagnosis skills

• Proven leadership and communication abilities

• Proficiency in Microsoft Office Suite
