Key Responsibilities:
• Work collaboratively as part of a dynamic team
• Provide quality in-home care to clients
• Ensure appropriate documentation and patient record-keeping
• Engage with patients to meet their health goals
• Travel to multiple homes as required in the community
Requirements:
• Active College of Nurses of Ontario registration
• Background check with vulnerable sector screening
• Own a vehicle with insurance for travel
• Proficient in English; bilingual skills a plus
• Documented negative TB test and liability insurance
