Make a difference as a Nurse Practitioner at Guelph Community Health Centre, focusing on specialized health care for vulnerable populations. Help clients overcome barriers to health and wellbeing with compassion and expertise.
This full-time role involves working within an interdisciplinary team to address health needs of priority populations including the homeless, newcomers, and those with mental health challenges. The Nurse Practitioner will deliver comprehensive assessments, diagnoses, and ongoing health care, utilizing a health promotion and disease prevention framework.
Key Responsibilities:
• Provide comprehensive health assessments and follow-up care
• Respond compassionately to diverse client needs
• Address barriers to health care access and stigma
• Maintain high standards of patient care per best practices
• Ensure ethical and confidential client interactions
Requirements:
• Registration in the Extended Class with the College of Nurses of Ontario
• Member of the Nurse Practitioners Association of Ontario
• Experience with communities facing health access barriers
• Proficiency in electronic health records and software applications
• Commitment to GCHC's Mission, Vision, and Values
