The successful candidate will work within the Supportive Addiction & Mental Health Housing program, providing essential medical and practical support to individuals facing complex addiction and mental health issues. Regular travel within the Guelph-Wellington area is integral to this position, allowing for hands-on support and collaborative efforts among multidisciplinary teams. Your commitment will greatly impact clients' housing stability and quality of life through compassionate nursing care.
Key Responsibilities:
• Assess and document bio-psycho-social needs of clients
• Conduct mental health screening and risk assessments
• Administer nursing interventions as ordered
• Facilitate medication management and support
• Engage in team meetings for collaborative care planning
Requirements:
• Diploma in Practical Nursing from a recognized program
• RPN in good standing with CNO
• Two years of relevant experience preferred
• Valid driver's license and reliable vehicle
• First Aid and CPR certification required
