In this full-time position, you will be part of an interdisciplinary team addressing the needs of vulnerable populations.
Your role includes thorough health assessments, disease prevention strategies, and crisis intervention. You'll focus on clients including Indigenous communities, 2
SLGBTQIA+, and those struggling with homelessness.
Key Responsibilities:
• Conduct ongoing health care evaluations for priority populations
• Implement strategies for health education and disease prevention
• Provide crisis intervention and supportive counseling
• Maintain confidentiality and adhere to ethical standards
• Collaborate within a team for comprehensive care delivery
Requirements:
• College of Nurses of Ontario Extended Class registration
• Experience in community health and social services
• Proven ability to manage complex client needs
• Additional language skills are an asset
• Familiarity with health service protocols and best practices
