Be a part of Guelph Community Health Centre as a dedicated Nurse Practitioner providing essential health services in Guelph, Kitchener, and Cambridge. This full-time opportunity centers on enhancing health equity for priority populations.
This ongoing role involves delivering comprehensive care to clients, especially those living with HIV and individuals seeking gender-affirming support. You will play a vital part in an interdisciplinary team focused on health promotion, disease prevention, and addressing complex health issues. Key Responsibilities:
• Deliver specialty health care to marginalized populations
• Perform detailed health assessments and crisis interventions
• Ensure accurate and timely electronic record-keeping
• Identify lifestyle-related health risk factors
• Engage in collaborative case discussions and inquiries
Requirements:
• Valid registration in the Extended Class with Ontario’s College of Nurses
• Proven experience supporting diverse health challenges
• Excellent verbal and written communication skills
• Familiarity with electronic health record systems
• Alignment with the values of Guelph Community Health Centre
