This role is pivotal for providing comprehensive primary care at Wellington Terrace Long Term Care Home. As the Nurse Practitioner, you will work within your legislative scope to deliver person-centered health assessments, develop care plans, and participate in quality improvement initiatives. Your leadership will guide the interdisciplinary team, ensuring optimal resident outcomes.
Key Responsibilities:
• Provide comprehensive primary care to residents
• Perform health assessments and medication reviews
• Prescribe therapies per medical directives
• Lead quality improvement initiatives
• Mentor and educate interdisciplinary team members
Requirements:
• Four-year Nursing degree and NP graduate program
• Minimum three years in a healthcare setting
• Current registration with College of Nurses of Ontario
• Certification in Basic Cardiac Life Support
• Experience in dementia and palliative care preferred
