This full-time position involves delivering specialized health services while collaboratively working within an interdisciplinary team. As a Nurse Practitioner, you will concentrate on health promotion, disease prevention, and the unique needs of marginalized communities.
Your role will actively contribute to reducing barriers in health access and improving patient outcomes.
Key Responsibilities:
• Provide specialty health care for HIV-positive and at-risk individuals
• Perform detailed assessments and health education
• Ensure timely and accurate electronic record-keeping
• Engage in client crisis intervention and supportive counseling
• Work within a health determinants framework
Requirements:
• Registered in the Extended Class with Ontario's nursing board
• Experience in community health and working with marginalized groups
• Skills in managing complex health conditions independently
• Strong written and verbal communication abilities
• Dedication to Guelph Community Health Centre's mission
