In this ongoing capacity, you will deliver holistic care targeting individuals facing health disparities such as those living with HIV and those seeking gender-affirming treatment. Working within an interdisciplinary team, you'll assess and manage client health concerns, ensuring comprehensive support and collaboration with other providers as needed.
Your role will be pivotal in not just treating conditions but also promoting overall community health.
Key Responsibilities:
• Offer comprehensive care to individuals living with and at-risk of HIV
• Conduct thorough assessments and treatment plans for clients
• Maintain compliant and current electronic health records
• Assess lifestyle and psychosocial factors affecting health
• Participate in interdisciplinary case reviews and discussions
Requirements:
• Registration in the Extended Class with the Ontario College of Nurses
• Experience with clients facing social health barriers
• Superior communication capabilities and multitasking abilities
• Proficient in relevant computer software and health records
• Dedication to Guelph CHC’s mission and values
