Key Responsibilities:
• Deliver care for clients with significant trauma
• Screen for mental health and chronic disease
• Complete primary care visits according to guidelines

Requirements:
• Bachelor’s degree in Nursing needed
• Valid College of Nurses of Ontario Certificate
• 2-5 years of nursing experience in hospitals or community
• Understanding of harm reduction and social health issues
• High proficiency in electronic medical documentation
