Mental Health Nurse Practitioner

Deliver mental health care in Guelph as a Nurse Practitioner using comprehensive assessment and medication management skills. Support young adults in residential settings.

This Nurse Practitioner serves young adults with serious mental illness through an innovative program. Working Monday to Friday, with occasional evening and weekend shifts, you will provide recovery-oriented care. The position emphasizes individualized planning, clinical collaboration, and long-term independence.

• Complete comprehensive health and mental health assessments
• Diagnose and manage acute and chronic health conditions
• Prescribe medications and monitor treatment plans
• Develop individualized care plans for clients
• Provide crisis intervention and risk assessments

• Current CNO registration as a Nurse Practitioner
• Experience in mental health or related clinical settings
• Strong diagnostic and medication management skills
• Experience with interdisciplinary healthcare teams
• Excellent communication and relationship-building abilities
