This full-time, permanent position involves working independently with clients experiencing substantial substance use and mental health challenges. You will conduct home visits, advocate for housing support, and coordinate care in collaboration with community partners. Weekend and evening availability is required, enhancing your engagement with clients in diverse settings.
Key Responsibilities:
• Conduct intake assessments and manage referrals
• Assist clients with move-in and access to health services
• Create personalized care and crisis response plans
• Perform monthly home visits and progress evaluations
• Document and maintain confidentiality standards
Requirements:
• College Diploma or University Degree in relevant field
• One year of experience in mental health or addiction
• Knowledge of case management systems
• Valid G Driver’s License and reliable vehicle
• Standard First Aid/CPR certification
Support clients in Guelph-Wellington by enhancing their quality of life through effective case management.
