Job summary

An exciting opportunity has arisen within Primary Care to work
as a Clinical Pharmacist with an Independent Prescriber qualification in our Churchill, Canbury, Orchard and Berrylands
Primary Care Network (CCOB PCN). We care for approximately 50,000 patients in
four practices across the borough of Kingston; this role would be based at The Churchill Medical Centre and The Orchard Practice.

This is a unique
opportunity for an enthusiastic and dynamic Pharmacist with excellent
communication skills to join us in our PCN. The successful candidate will work
as part of a multidisciplinary team within our practices and liaise with
external healthcare professionals from community and hospital settings. The PCN
can also offer support within a wider pharmacy team including prescribing
clerks, pharmacy technicians and another clinical pharmacist employed across
our practices.

Main duties of the jobThe role will have a strong patient facing focus, with the successful
candidate undertaking the following areas of work:

Medication reviews to
proactively manage patients with complex poly-pharmacy

Responsibility for areas of
chronic disease management, dependent on experience

Provide support on
prescription and medication queries, providing clinical guidance to the
pharmacy technicians and prescribing clerks.

Provide clinical leadership
on medicines quality improvement

Contribute to managing some
aspects of the quality and outcomes framework

Work with Kingston CCG
pharmacists on the local medicines optimisation framework

You will need to have a combination of at least two years post
registration experience, an Independent Prescribing qualification, a degree level qualification, Registration with the
GPhC and Professional Indemnity membership. You will also need sound clinical
knowledge and computer literacy.

operational efficiencies so requires self-motivation and passion to deliver
high quality patient care within general practice. The successful candidate
will have excellent organisational, interpersonal and communication skills
together with a good working knowledge of primary care.

GPhC and Professional Indemnity membership. You will also need sound clinical
knowledge and computer literacy.
