• Provide 1:1 care following the client's plan of care, maintaining dignity and independence
• Administer medications, feedings, oxygen, ostomy care as needed per the plan of care
• Accurately document nursing actions, communicate with family, case manager, and physicians, and inform all parties of changes in the client's condition
• Coordinate or order supplies, medications, and feedings to ensure adequate amounts are on hand
• Ensure all patient needs are met per physician instructions

• LPN/LPT/LVN degree from an accredited nursing college with current unrestricted license in the state of practice
• Valid driver's license with acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ year of nursing experience preferred
• Home health care experience preferred

The position will be initially employed by Amedisys until January 1, 2026, with all onboarding and benefits under the Amedisys employee benefit plans. On or before that date the employment will transfer to Adoration Home Health / Bright Spring Health Services, where they will supervise and direct the work of the candidate.
