Physical Therapist – Gurnee Rehab Services
Illinois Bone & Joint Institute, LLC is hiring a Physical Therapist for our Gurnee Rehab Services location. This position reports to the Facility Manager and focuses on providing superior outpatient rehabilitation services through a collaborative care model with physicians to optimize patient function.
Job SummaryThe main focus of the position is on providing superior outpatient rehabilitation services through a collaborative care model with our physicians to optimize performance and enhance the quality of function for our patients.
Responsibilities- Provide exceptional patient care
- Interpret physician orders, perform initial evaluations, and establish treatment plans and functional goals
- Update and amend treatment goals as necessary during the episode of care
- Document patient status in compliance with IBJI rehab guidelines
- Determine appropriate discharge timeline and follow‑up plans
- Supervise Rehabilitation Technicians and patient care staff as needed
- Maintain timely documentation of all treatment (initial evaluations, daily notes, progress notes, discharges, HEP)
- Stay current with billing policies and demonstrate ethical billing practices
- Maintain an active state license by earning appropriate CEUs through external courses and IBJI‑sponsored educational opportunities
- Develop positive relationships with the physician teams at IBJI
- Actively participate in the patient medical team: office visits with physicians, contact with case managers, etc.
- Participate in programs sponsored by IBJI: mentoring, clinical educator, special interest groups, community outreach
- Graduate from an accredited Physical Therapy Program
- New graduate physical therapists eligible to apply
- Licensed or eligible to sit for boards in the state of Illinois
- Ability to work in a fast‑paced environment
- Professional and effective written and verbal communication skills
- Ability to lift greater than 25 pounds
- Demonstrate good understanding of proper body mechanics for lifting, pulling, pushing
- Ability to stand and sit for sequences of time greater than 30 minutes
- Ability to work on a computer for lengthy periods during the day
