Hartekamp Groep zoekt een Casemanager Gehandicaptenzorg voor 28-32 uur per week. Je ondersteunt teams bij complexe zorgvragen en coacht bij herstel en verbetering van de zorgrelaties met cliënten.
Je past Heijkoop-methodiek toe, geeft trainingen en organiseert bijeenkomsten zoals Ontdekkend Kijken. Een HBO-diploma en flexibiliteit voor ochtend/avondwerk zijn vereist, inclusief rijbewijs en eigen auto.
