ViVa! Zorggroep zoekt een coördinerend verzorgende IG in de thuiszorg voor het wijkteam Zuid-Kennemerland. Je coördineert zorg en verzorging van cliënten, bouwt relaties op met cliënten en naasten en helpt cliënten zo lang mogelijk thuis te blijven wonen.
Werkzaamheden variëren van het verhelderen van zorgvragen en signaleren van veranderingen tot zorgplanbesprekingen en ADL-ondersteuning. Je werkt 0-32 uur per week; de exacte werkdagen stemmen we samen af. Solliciteren kan via onze recruiter.
