• Provides IMR and crisis intervention services.
• Assists with food shopping, laundry and daily living skills.
• Implements prescribed treatment plan and assists in the development of IRPs.
• Must be able to work independently and travel to consumers’ residencies as required.
• Monitors medication in the consumers home.
• Completes documentation as required.

• BSW in social work, BA/BS in psychology
• BA/BS in criminal justice, or a related field with two years of experience in mental health
• Valid Driver’s License

$43,650 annual salary

Monday to Friday, three days 9am—5pm and two days 1pm-9pm
