PHARMACY TECHNICIAN NAVIGATOR I – MEDS TO BEDS (FULL TIME)

OverviewThe ambulatory pharmacy navigator program helps HMH patients receive their discharge or specialty medications quickly and efficiently, facilitating smooth hospital discharge and home recovery.
Responsibilities- Perform all duties of an Ambulatory Pharmacy Technician plus additional tasks.
- Greet patients admitted or scheduled for clinic appointments and explain available pharmacy services.
- Enter prescription orders and obtain patient authorization for ambulatory or specialty pharmacies.
- Collaborate with floor nurses, discharge planners, case managers, social workers, physicians, APNs, PAs, medical residents, unit clerks, and other teams to ensure timely medication delivery.
- Determine HMH pharmacy selection for patients scheduled for discharge or clinic appointments.
- Identify required information sources (e.g., insurance coverage) for prescription processing.
- Troubleshoot patient medication access issues related to insurance or financial coverage and resolve them.
- Adhere to HMH pharmacy and federal/state privacy and compliance policies to protect patient confidentiality.
- Facilitate prescription fulfillment by in‑house pharmacy and coordinate patient pickup or delivery to care areas or home.
- Ensure patients receive an offer for pharmacist counseling and provide contact information for follow‑up questions.
- Distribute medication‑related educational materials and coordinate counseling with staff pharmacists.
- Assist Ambulatory Pharmacy staff and perform duties of an Ambulatory Pharmacy Technician as needed.
- Maintain a safety‑first work ethic and report medication errors or variances per HMH Pharmacy Policy.
- Perform other duties or projects as assigned.
- High school diploma, GED, or equivalent.
- Excellent written and verbal communication skills.
- Proficient computer skills (Microsoft Office, Google Suite).
- Retail or specialty pharmacy experience.
- Knowledge and experience with PBM claims and coverage.
- Pharmacy Technician License.
- PTCB or equivalent certification within 180 days of employment (exceptions for pre‑2024 hires or students).
Base rate: $26.71 per hour (minimum). Additional factors may influence final offer, including market data, experience, education, certifications, location, and internal equity. Shift differentials may apply for evening, night, or weekend work. Performance‑based incentives, bonuses, or commissions may also be offered.
