Position OverviewWe are seeking a skilled Advanced Nurse Practitioner to join our team. We are a partner with Holy Name Medical Center and continue to grow our footprint in Northern New Jersey.
Responsibilities- Manage both acute and chronic conditions, with a focus on continuity of care across rehab, long‑term care, and outpatient settings.
- Provide primary care services to patients across the lifespan in a variety of settings, including outpatient offices, rehabilitation centers, and nursing homes.
- Perform comprehensive physical examinations and obtain detailed medical histories.
- Order and interpret diagnostic tests, including laboratory work and medical imaging, to support accurate clinical decisions.
- Prescribe and manage medications and treatments within the NP scope of practice.
- Conduct health coaching and patient education, promoting wellness, preventive care, and chronic disease management.
- Collaborate closely with physicians, therapists, nursing staff, and other healthcare professionals to develop and implement coordinated patient care plans.
- Provide specialized follow‑up care in rehab and nursing home populations, addressing issues such as mobility, pain management, medication compliance, and discharge planning.
- Utilize electronic health record systems (e.g., eClinical
Works) to accurately document encounters, coordinate care, and track patient progress. - Maintain high standards of infection control and aseptic technique in all procedures and care settings.
- Support transitions of care by communicating effectively between rehab/nursing facilities and outpatient office staff.
- Valid Nurse Practitioner license in the state of NJ
- Master's degree in Nursing with a specialization as a Nurse Practitioner
- Certification as a Nurse Practitioner in the appropriate specialty area (e.g., Family, Pediatrics, Adult‑Gerontology)
- Experience in primary care or a related field preferred
- Strong clinical skills and knowledge of evidence‑based practice guidelines
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members
