PurposeWe are looking for an experienced Nurse Practitioner to provide primary and palliative care to homebound patients in Passiac/Hudson County, NJ. You will apply medical techniques under physician supervision and care for a diverse set of patients with chronic diseases.
Responsibilities- Provide high‑quality care to primary and palliative patients in and around Bergen/Hudson County, NJ
- Assess patients by performing physical examinations, obtaining and updating medical histories, and developing the best care plans
- Communicate effectively with office staff and clinical operations across internal platforms
- Provide patient-centered care to a panel of patients
- Attend quarterly medical staff meetings
- Develop impressions, diagnose, and create treatment plans
- Foster trust and consult with patients, families, and caregivers during visits
- Order diagnostic testing as medically necessary
- Prescribe medications and appropriate interventions
- Board Certified and a valid NP or PA certification in New Jersey
- Strong clinical assessment and diagnostic skills
- Positive demeanor and strong interpersonal skills
- Ability to problem‑solve and operate independently
- 1–2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)
