Purpose

We are looking for an experienced Nurse Practitioner to provide primary and palliative care to homebound patients in Passiac/Hudson County, NJ. You will apply medical techniques under physician supervision and care for a diverse set of patients with chronic diseases.

Responsibilities
  • Provide high‑quality care to primary and palliative patients in and around Bergen/Hudson County, NJ
  • Assess patients by performing physical examinations, obtaining and updating medical histories, and developing the best care plans
  • Communicate effectively with office staff and clinical operations across internal platforms
  • Provide patient-centered care to a panel of patients
  • Attend quarterly medical staff meetings
  • Develop impressions, diagnose, and create treatment plans
  • Foster trust and consult with patients, families, and caregivers during visits
  • Order diagnostic testing as medically necessary
  • Prescribe medications and appropriate interventions
Qualifications
  • Board Certified and a valid NP or PA certification in New Jersey
  • Strong clinical assessment and diagnostic skills
  • Positive demeanor and strong interpersonal skills
  • Ability to problem‑solve and operate independently
  • 1–2 years of primary care experience with geriatric patients (hospice experience highly preferred but not required)

Salary Range

$$+ (depending on experience and other incentives)
