The Hospital at Home program delivers acute‑care nursing in patients’ homes, allowing RN staff to handle the intake/admission process as if they were in a hospital. This role supports our mission to transform healthcare by providing high‑quality care directly to patients’ homes.

• Deliver nursing practices based on current evidence‑based research, policies, and standards of care to provide safe and individualized patient care.
• Communicate continuously with patients, nursing colleagues, and other team members regarding patient status, rendered care, and treatment plans.
• Administer medications using bar‑scanning and documentation technologies to ensure safe delivery.
• Document patient assessments, plan of care, interventions, and evaluations electronically at the point of care.
• Provide patient‑ and family‑centered education to optimize health, wellness, and disease management.
• Coordinate multidisciplinary discharge planning to ensure safe transition and continuity of care.
• Perform home visits and facilitate the intake/admission process for the Hospital at Home program.

• Current and valid New Jersey RN license.
• BLS/CPR certification by the American Heart Association required within 60 days of hire.
• BSN required.
• 2 years of acute‑care experience preferred.
• Comfortable with home visits to patients’ residences.
• Reliable transportation method.

Starting at $47.39 hourly, subject to the New Jersey Pay Transparency Act. The posted rate is a reasonable, good‑faith estimate of the base pay.
