RN Case Manager - TCI-Cure and Beyond-PT

The RN Case Manager oversees and directs patient care activities for a specific group of patients, revising care plans based on guidelines and protocols. This role is ideal for experienced nurses who excel in collaboration and communication within a multidisciplinary team.

• Pay: $48,505/year
• Location: Hackensack, NJ
• Facility: Hackensack Meridian Health
• Schedule: Part-time, Day shift

• Collaborate with physicians, nursing staff, and multidisciplinary team in assessing, planning, implementing, and evaluating care.
• Function as a clinical resource for nursing staff and physicians in identifying problems and developing solutions.
• Develop and maintain a communication system for timely information transmission to the care team.
• Revise the plan of care in collaboration with patients, families, and the multidisciplinary team.
• Monitor documentation in the medical record to ensure patient outcomes are achieved.
• Establish a system for coordinating patient care across geographic areas.
• Communicate and collaborate with the multidisciplinary team involved in patient care.
• Collect, analyze, and share data to assess compliance with established guidelines.
• Educate the multidisciplinary team on compliance with current guidelines.
• Participate in continuous process improvement efforts to enhance patient outcomes.

• Bachelor's Degree in Nursing from an NLN/AACN accredited program required.

• 2 years of experience as a registered nurse required.

• Current NJ State Professional Registered Nurse License required.
• Certification in area of specialty required.
