Atlantic Health System is seeking a per diem Social Worker (MSW) for Case Management at Hackettstown Medical Center to work 8AM-4:30PM including one weekend every four weeks.

• Discharge planning from an acute care hospital.
• Supportive and therapeutic counseling.

Minimum Education:

• MSW and LSW

Minimum Experience:

• 1 year in a medical setting (acute care hospital) or 1 year externship would be considered.
